Employee Campaign Pledge Donation Form
Your participation in the Employee Giving Campaign means so much to us. As the campaign progresses, we look forward to sharing what your support has made possible. If you are a Saint Agnes or Trinity Colleague, please fill out the Pledge Form below.
If you are a Provider or SAMP Colleague, please go back and visit the other link that will direct you to the credit card monthly deduction form.
Donation deductions will begin with the first pay date in September and will run for 26 pay periods.
If you are currently in a pledge from 2025, that would be completed first then the new pledge will begin.
You can pick up your swag items from the following locations and times below. You can also pick them up at the Foundation Office at 1111 E. Spruce Ave #910 (559) 450-2040 Monday through Friday 9:00 a.m. to 4:00 p.m.
